General Price List
General Price List - This is a full itemized list of goods and services that we provide as a funeral home and the charges associated. These prices are effective as of January 1, 2023 but are subject to change without notice. Provided are links for both Doc and PDF file types.
Information Sheet
Information Sheet - This helps gather information that is required on the death certificate and placing an obituary in the paper (if desired). Provided are links for both Doc and PDF file types.
Death Certificate Checklist
Checklist for Certified Copies of DC - This sheet will help determine how many death certificates you may need. In Arkansas, certificates are $10 for the first copy and $8 for each additional. If the individual was a veteran, then he or she receives the first copy for free. Provided are links for both Doc and PDF file types.
Cremation Authorization
Cremation Authorization - An individual can sign his or her own authorization. If unable to, then the legal next of kin(s) is/are required to sign. It also requires two witnesses. Provided are links for both Doc and PDF file types.
